The "yellow card" and the international agreements already exist. I think it is only necessary for the yellow fever vax any more. The other vax information is just for information only.
The yellow fever needs quite a bit of info on you and the vaccine, plus signatures and a stamp from a authorized vax center. So, the framework is there. Will there be fraud? Sure, but a lot of risk to take to avoid a pretty cheap shot that could save your life.

It is recognised internationally and may be required for entry to certain countries where there are increased health risks for travellers. The Yellow Card should be kept in the holder's passport, as it is a medical passport of sorts.
Some employers also use it to make sure you are up on your shots before company travel. The dog has hers, I have mine. Canada really wants to keep rabies out.
