I think you're mistaken. You have to turn FMM in when exiting Mexico. It is a single entry visa, "multiple" written on it means that it can be used for different purposes, but it is a single-entry. But... Nobody cares to return it unless they are flying out, and Mexican border agents don't care much. It is often difficult to return it when you're driving out - there is no dedicated booth or a space to pull off and walk across the lanes from North-bound to South-bound lanes. At least, in Tijuana and Otay.
Relation of status to insurance coverage is not clear. I would expect insurance coverage denied if you don't have a valid FMM, but I don't see reports of this happening.
