Years ago we bought insurance at the border when we crossed near Chetumal. Last year at El Ceibo there was no way place to buy insurance. We only spent a week and stayed in one place the whole time. We never did get insurance. I don't think it's required (but don't quote me on that), though obviously always a good idea.
