They work great and are cost effective in apartments (much more efficient than a PTAC) or smaller homes, <1000 sq ft.
For optimal performance, they require an air handler in almost every room ($$$). Although most mini-split pumps (the part that sits outside) can handle 2-4 air handlers, larger houses would need more. There is quite a bit of plumbing required for each air handler which is why they are typically mount on an exterior wall so all of that can be routed outside and the covered.
Big houses/buildings can actually have 2 pumps each with multiple air handlers. The air handlers can be slaved together to condition one large room or operate independently.
Besides being more efficient than tradition AC/heat pumps, another win is being able to only condition rooms that are occupied. No need to keep your bedrooms at 75F during the day when you know it can be quickly cooled down before going to bed.
