Recreation.gov - once you place the site in your cart, it will hold the site for approximately 15 minutes. If you don't complete the reservation within that time, it will release it. That's a fair system - you don't get to block a site (or multiple sites) for an unlimited amount of time. But you have plenty of time to get through the reservation process once you pick your site.
We were already physically on the site and paid up for 3 days. The P.O.S. machine didn't talk to Recreation.gov so they didn't know the site was occupied; in addition it was a first come first serve site. Neither the park service employee or the host had any idea it had been reserved until the people showed up on our last night there.
