If you're saying yes to the customer who prefers Amex, you can have many more opportunities to earn back those fees and more. If you say no, you may have a one-and-done customer (or even one that walks away without completing the transaction).
The retail companies I've worked for (including the current one) and accept Amex are more concerned about me helping the customer buy more items than in having me try to manage back-end costs. All 3 firms I've had first-hand experience with do not have PIN-pads at the registers, even though they'd allow you to further reduce the processing costs when accepting debit cards.
