The interior of my door panel has multiple cracks and needs to be replaced. I was hoping that it was simply an FRP panel layered over insulation, but it looks like the insulation and the panel are bonded together as one.

You will have to make your own, doubt you will find something preinsulated, the door has wood framing as a structure so there is wood and foam insulation between the two layers of FRP..
In reality you don't need the insulation since you should be able to reuse the existing insulation, unless you have to scrap the entire door..
If you do scrap the door you will have to frame out a door then cover it with FRP, then glue the insulation then cover the other side of the door.
You can buy FRP at Lowes or home depot in the paneling section, this FRP is thicker than what is used on RV doors. Typically is used in commercial applications like restroom walls and any other place where high moisture exposure would ruin drywall. One side has raised bumps (finished side) and the other side is flat (goes towards wall).
Can be glued to your door via paneling adhesive, your choice if you want to glue over top of the existing panel or attempt to remove the damaged panel. I suspect attempts to remove the existing paneling may cause collateral damage to the rest of the door if not careful..
One 4x8 FRP sheet runs about $30..
