2004 Coachmen Somerset Dreamcatcher 358RLTS Slide Motors

My slide motor is starting to fail on my big slide (dinette, TV, and office). It often smells like smoke and frequently stops. I have to stop and let it cool down multiple times. It's been properly lubed and doesn't bind, and everything looks like it's property adjusted too.

I need to know what slide motor it is, brand and model, or a suitable replacement. I'm partially disabled so I'd like to be able to order the replacement without having to work for hours getting to and finding the motor.

Please, can somebody help? I usually have an awful time finding parts for this thing because it's apparently not like a truck where you can rattle of year, make, and model and look things up.
