Not being there and seeing exactly what was going on makes it hard to diagnose but, I would suspect that it came from the factory that way. As stated, cable or valve not aligned properly! Before the failure did the valve action feel "stickey"? I hope the technician knew what he was doing and made sure everything was aligned properly when they did the repairs!
