NorthAmerican and MonteCarlo produce mostly for the Carnie and Traveler folk. So they don't worry about things like fitting into public campgrounds.
Personally, if I wanted a custom (at custom prices) I would talk to New Horizon. Mind you they will not be cheap. But they will produce exactly what you want.
Plus they actually have "cred" in the full-time RV world.
