I can't remember much from my military truck driving training, too many years gone by, but what I do remember is that you always used your mirrors and NEVER NEVER stuck your head out the window to have a look-see. I guess that is why it has never mattered to me where my hands are on the wheel because I am not switching my view back and forth from looking in the mirror then trying to look back out of the window.
