Op I have basically the same truck and B&W hitch as you. For one raise your drivers seat to the highest position. Also remove the center headrest on the back seat. If you still can't see your hitch scoot up in the seat as you look over your shoulder.
When you unhitch hang a string to the ground with some kind of weight on the end then clamp it to the kingpin, now coil it up and leave it clamped so when you go to hook up you drop the weighted end to the ground then raise the front of the RV so the string just touches the ground now you are at the perfect height to connect.
If you still have trouble seeing it add a small antenna to the front of the hitch centerline.
As far as a ladder I am in the process of coming up with a step that can be pulled out just in front of the rear tires so I can get to the tool box and the hitch easily. But for now I just stand on top of the outer rear tire.
