I am wondering how close the Reese hitch (center of pin) was to the back of your cab. Some non-slider hitches can be set back over axle center, allowing just over 40"s between pin center and cab. You also have the Andersen option, with a coupler that can be reversed, allowing more clearance. The Pullrite is popular, but heavy, and alignment issues, like you mentioned.
