I will repeat my earlier post:
Looks to me that they had a round pipe on the forklift (fork) that just goes over the king pin to move trailers around and it tried to go up between the wedge and the king pin.
The marks on the wedge are curved, indicating something round did it, like a pipe/round tube mounted vertical on the forklift.
