The 399TH should be a great unit. I think you will be very pleased with Grand Design. We have NO regrets about them. Our 385TH is working out very well, and we are full-timing in it.
As for trucks, we went with a LB. It's the most versatile in my opinion. More room, and you won't need to even consider a slider hitch. I also prefer the size when towing something as long as these are.
We went with a crew cab because there is plenty of room in the back for people, and towing the 399TH (385TH in our case) you will have all of your "stuff" back there. There is plenty of storage under the seat for the incidentals.
For the hitch, if you go with a RAM your best choice is to make sure it has the factory 5th Wheel/Gooseneck Towing Group and use the B&W RVK3600. It's what we went with. http://www.ramgunner.com/rvk3600.html
If you go with a shorter bed and think you would be more confortable with a slider, CURT is making a set of legs to drop into the RAM system that work with their sliders. I have a list of everything we know of for the RAM system at http://www.ramgunner.com/ram5thwheel.htm
