We had almost the same thing happen last fall on our 44' voltage toyhauler, only 6 moths old and 2000 miles later headed to AZ. for the winter. We had stopped for fuel and checked everything out and headed down the road. 2 miles later we noticed smoke out the drivers side window, we pulled off and noticed the wheel at an odd angle. Jacked it up and the wheel fell off into my legs. Luckily we had just passed the Flagstaff, AZ. camping world. They were SUPER to us, but we had to sit a week because ours was under warranty from Lippert and they would not authorize a repair but sent a whole new axle to camping world. Camping World at Flagstaff,AZ. was outstanding to us while we waited on the axle. They also said they had not seen anything like this before and related ours to a bad bearing.
