The further south you go, the more options you will have as to places that are open year round. FL and South TX are common places for the snowbirds to get away from the cold. Now granted you will have the opposite problem in the summer. Need to run the AC all the time, or most of the time.
If the programs in the South are quality programs, then I would definitely look at one of them. Since you were talking about a program in the medical field and you mentioned NC, so I would guess that is in the Raleigh/Durham/Wake Forest Area and I would guess it would be a good compromise. My DD and SIL live there, and they have only had a few days of extreme cold. You could make do in RV for a few days of extreme cold much easier than the extended cold of Pitt and other points in the north. Their summers are hot, but not as extended heat of summer as south FL or TX.
