Surely someone in your IT department can "construct" a card that could be printed on cardstock at the same time the "Dec" page and Terms & Conditions are given to the customer. Heck, even 2 could be printed so one could be kept in the rig and another kept in the person's wallet. The customer could certainly cut it out.
It's not rocket science. It's thinking about putting the customer above the bottom line.
