If you want to do it yourself, you can get most of the supplies from Amazon.com.
I did one last year, and documented with pictures at link below:
Dual Pane Window Repair-My Way
If I were doing it again, I would not reuse the old window spacer. New window spacer contains some chemicals to help keep the inside of the window dry. I could not find 3/16th spacer, so I reused the old spacer. Almost all of the time, my repaired window is completely clear. I've had one time where the sun hit the window just right along with the temperature caused a little fogging inside the window. After a year, the window is still clear.
If you don't want to buy supplies from Amazon.com, Dave Root sells a kit for the window repair. You can see the kit and additional info at the link below.
