I had a similar issue that I messed with for some time changing filters and treating for algae, etc. with what seemed to be positive results for the short term and then it would come back.
End result was a fuel delivery problem from a failing fuel transfer/lift pump. Replaced it and have traveled over 3,000 miles since with no issues. The fuel delivery codes showed up when we hooked the computer to it and like you there were a lot of possibilities.
According to the techs I discussed this with, the consensus seems to be that the 2003-4 time frame is when the change to low-sulphur fuel happened and the lift pumps from that era are failing because of seal issues from compatibiliy with the fuel of today. I can't say that I know this as fact, simply relaying opinions from talking with people who I suspect may know better than I do.
