For 30 amp that is not a possible scenario. Simply check the voltage and polarity before you plug in.
Yes it is needed as it protects against other problems especially varying voltage. And I don't know of anyone that watches a voltmeter all day long. :(
I also had 160V due to a bad neutral on 30A. Somewhat unusual as it was a problem in the CGs 240V distribution system. So it can and does occur on 30A plugs.
And currently I have limited use of a 20A plug that has an intermittent open ground.
In both of these cases and others the PI unit protected the rig.
