I would remove the cap where the bar travels through to see what is going on... You will probably need to replace the handle.
I have no idea what you are describing here. There is no "cap" that can be removed anywhere on the exterior of the valve. Maybe you're referring to a different brand or model? I don't know what brand I have, there is nothing written on the handle...
