I have yet to hear of anyone having air loss issues with Borg/Tire Man metal stems. Many Sprinter folks use them.
For the OP, I had no issues with the stock extenders/stems when using V2B air through caps and a dually chuck. I have since got rid of the hub caps due to their noise.
