You are correct that the outside tire does almost a 180* turn but it was built with this bend in it so I'm not worried about it. I have had other valve extenders installed on dually trucks and that is how they are all done.
My rear stems are about that long also and they do come with a flow through valve cover. I didn't need them on the rears (or the fronts) so I removed them. The stem still sticks out far enough that I can mount my TPMS on it without any problems.
I would still install a TPMS on your rig because just because you can air them up doesn't mean as you are driving down the road you might pickup a nail and have a tire go down and you are not going to know about it. I have ran TPMS systems for over 8 years now and wouldn't be without them. I have them on my pull behind trailers also that report to my controller in the cab. When I had my 5th wheel I had an alarm on time running down the road so I pulled over and the tire was down some and was still leaking air. On one tire the nut on the valve stem came loose and the air was leaking out around it. This tire had never been removed from the rig since it was new. So for some reason probably no lock tite used it just came loose. I tightened it down and filled the tire up and was on my way. If I hadn't had a TPMS I would most likely had a blow out down the road as the tire lost more air and heated up. These were load range E tires with 80PSI in them. The pressure had dropped to 70 PSI when it alarmed to me. At that time the system paid for its self as far as I'm concerned.
Also you don't want to put a TPMS system on rubber valve stems. The weight on the outside of the stem will destroy the valve and you can have a tire loose its pressure in a few seconds. All of the TPMS systems that I have looked at suggest that you have the stems replaced with metal one.

I kind of anal about things (or at least that is what the wife tells me and I guess after 43 years she should know) and if there is anything I can do to make sure I don't have problems on the road or some time down the road I'll do it.
