Two ideas.
First the 3-valve bypass system consists of a valve in the COLD line to the heater (Obviously this is open, as it should be) one in the HOT line from the heater (likewise) and one before the first two in a tie-line (Crossover or bypass line) that SHOULD BE CLOSED.
Second is a 2 parter, A and B, both are the same save for one word.
The shower.. A:Outside, B:inside, Many have a cut off valve in the head and the #1 cause of the symptoms you describe is using the shower, usually the outside which is why I ranked it "A", and cutting off the head, but leaving the mixing valves open.
