I'd call it the "can you wipe your butt" test. Anyone can just sit on the commode in these vans, it's the rest that can be an issue.
Some of the bathrooms are just head scratchers. Why put fancy woodwork in them that need protecting? Why even put a sink in there that doesn't fold away?
The newer vans like Transit and Promaster have more upright side walls than the Sprinter. When there is a better selection of vans, you might find it easier to stand up in a side bath setup.
I think the best bathroom I've ever seen is in the Shire conversion in the UK.
It spans the full width of the rear of the van, but it's fairly narrow, with the shower pan in the center. I've posted it before.
