As a former insurance adjuster from years back, I can tell you ladders are huge liability issues. You are probably correct. The MH manufacturers probably figured it's better to leave the ladder off which leaves the owner to figure out a way to get on the roof and if they fall they sue whomever built the ladder they did use. But I can see a good (or sleazy) lawyer going after the MH builder and claiming their client would not have been on the portable ladder had it not been for the MH not having a fixed ladder and therefore it's still their fault.

Now, you two have stated what I believe is the real reason behind the disappearing manufacturer mounted ladders. Lawsuits! Many of which are unwarranted. People looking for something for nothing though their own stupidity or carelessness encouraged by sleazy lawyers. They advertise on television like a bunch of hucksters.
