Dog ID Card for Travel

I have seen the cutesy state ID cards but I am looking for veterinarians that offer ID cards that can be used in RV parks that require documentation of vaccinations when checking in or possibly traveling to Canada?

We will be in Arizona this winter anyone there have a veterinarian in the East Valley area that offers these ID cards?
  • Just an FYI --

The only legally recognized proof of rabies vaccination is the certificate your vet provides that lists identifying information about the vaccine used (manufacturer, batch number, etc.) and identifying information about your dog, and that is dated and signed by the vet. An "ID card" or rabies tag are not legally valid forms of proof.

I've never seen anything like this, but I like the idea.

I first saw one of these when visiting friends in Georgia before we got our dog. It is good advertising for the Vet's office and the micro code can be scanned just like a chip. This may be something for a larger practice as I am sure the scanning software and bookkeeping system is an additional cost to the office.

Just rec'd a PM of an Veterinary office in Mesa AZ that offers pet id's: http://altamesaanimalhospital.com/services/pet-id-cards

Thank you for the info. Maybe this can change down the road if their is enough interest?

One of the features of an the ID card is easy fit in the wallet and quickly accessible phone number. though the vet's phone number could already be in your phone contacts.
  • Easy to make yourself.
We take a picture of the dog then print it out about 4x6. Write our phone number on it and the dogs name.
    Take the shot record card.
Lay them both on the printer and print several copies. Now you have a photo ID for your dog. We show it when entering Canada just like our passports.
If they should get separated from you, you can show the picture around, give it to the local police, etc.
  • There are dog passports that are legal documents, just like passports for people. I have a client who takes her dog to Greece a couple of times a year and the US forms got to be so difficult to get and properly filled out that she opted for the European Pet passport.

The EPP is a great idea. Wish the USA would make one available.
  • Our vet here in Mass. does them. We just returned from 8 months on the road, carrying all Chloe's records as well as the card and only were asked for rabies cert one time by a groomer. No campground, private, municipal, county, state or federal ever asked about her.
