I'm going to say that I think you just know when its time. That said, did they do bloodwork? Did it come back clean? Maybe the missing the box is the sign of some other issue.
For instance, we have a short hair poly-dactyl that is the king of the house. One day out of the blue, instead of being his normal nosey self, he was sleeping in places he normally doesn't, staying away from family that was over (which he doesn't do), etc. Thought it was odd but ok.
Later that night he spends an unusual time grooming his..er..area. Took him to emergency...said seemed fine, bloodwork ok, but constipated. Gave him enema. Next day we observe him in the litter trying to pee..trys like 10 times and cant go. Take him to vet, can't express bladder..says is blocked.
He is better now, but its taken a few weeks to get through his procedure, meds, they said he had a tapeworm (I think they are nutz), then he had a eye problem, then kitty pink eye...3 weeks ago, no problems whatsoever.
We also have a much older female. She has been missing the box for I don't know how long...thing is..she just doesn't want to get in it..isn't comfortable.
Saw a thing on TV..cat show lol, with a custom box. So I made her one. Its huge, but nothing to step over, which we think is what causes her pain and not to want to use it. Much less missing the box now.
He does look very good for being a 20 yo.
