You could try a different vet, they might be more able to diagnose him.
My experience with older cats is that often, if they get sick, they have an attitude of, 'Well, I guess it is just my time.' Then, if they get treatment and get better, they figure it was not their time after all.
I guess what I mean is, after a certain age I think they are ok with going any time, and see their remaining days as a bonus. My grandma was like this too. She talked about it. The last 5 years or so of her life, she was enjoying life, but also feeling like she was ready to go whenever it happened. Once she was woken up by an earthquake, on vacation in Mexico, and she didn't panic, she just thought, 'I guess this is it.' But then it wasn't.
I still miss the heck out of her, and of my cat who died last summer.
The other thing I tell myself, when stressing over these decisions about pets, is, 'Whatever I do, it's better than being eaten alive by a coyote, which is probably what would happen in the wild.'
The fact is, it is just HARD to make this decision for another creature. You rarely get to be 100% sure you are making the right one. But kitty knows you love him, and are doing your best. He knows you pretty well.
