As Pawz suggested, I/D would be a great choice. The pancreatitis is a more important dietary issue than either the heart, diabetes or Cushings.
At this point, I wouldn't be concerned yet about long term diet considerations. The first issue is to make sure the pancreatitis is under control or gone and the diabetes is managed. Then see if Cushings is indeed present and a cause. If so, that needs to be addressed, THEN consider long term dietary issues. Until then I/D (or similar prescription food or home made diet) would be my choice.
