Has your vet completely ruled out a kidney infection?
I fought with a diagnosis of UTI and treatment of tons of different very harsh antibiotics with serious side effects to both the kidney and liver to no avail.
I brought in a urine sample and had requested it be sent out for culture. When a culture is requested they also will give you results as to which antibiotic it is susceptible to.
Results Kidney infection leading to kidney failure. And susceptible to all things amoxy. :R
Three days slow drip iv and meds and he finally turned around.
I vote for cultures all the time. Gives you the results and what to treat it with. The added cost of a culture? Will be WAAAY cheaper than going thru 20 different meds thrown at you dog to see which one sticks while he is suffering.
