You keep the modem from TWC that has the telephone device, but the internet modem part of it is disabled by TWC. Since the internet part is not usable you no longer have to pay the rental fee, even though you still have the modem. Yeah, it makes no sense.
I believe that's due to some government rules relating to telephone service and what can and can't be charged to the customer to provide voice service.
The eMTA (embedded Multimedia Terminal Adapter) portion of the cable modem is technically a VoIP converter. It runs straight off of the broadband cable signal. In theory, this can sound better than conventional telco POTS. In practice, it usually doesn't due to delay and jitter down the line.
If you use a separate VoIP provider, you can get a VoIP-to-POTS (Plain Old Telephone Service) converter that will let you plug your conventional phone device in and run it over the Ethernet network that you plug into the Ethernet portion of your modem. You can get VoIP service from conventional providers for as little as $60 a year if you look around. Then you don't need the eMTA modem part.
