Instead of duck tape, I suggest Gorilla tape. It's similar, but better, and less messy as well.
Pretty much what I carry is what's been listed here: tire changing stuff (including the very important wheel chocks), small 120V compressor, screwdrivers, pliers, adjustable wrench, cheap multimeter, level, tire gauge, gorilla tape. I do have a few other minor odds and ends: a tire valve core tool, a utility knife (most handy), a caulking gun that I hope I never need to use while on the road, hatchet and bowsaw (more for camping than for repair work), a fuse puller/tester, and probably a couple of other things I'm not remembering at the moment.
Anyhow, to limit it to five, I guess I'd choose:
1. #2 Robertson screwdriver
2. Adjustable wrench
3. Swiss army knife or Leatherman or something similar
4. Tire pressure gauge
5. Gorilla Tape
