Frankly, shower and toilet are not minimalist, and the only thing with those facilities that will get close to 20 MPG would be a van conversion (type B motorhome) on a diesel engine platform, most likely Daimler's Sprinter.
Compare a $100,000 to $150,000 20 MPG RV purchase cost to what you might save on fuel costs, 12-16 MPG towing something light and cheap vs driving something 18-20 MPG that is very expensive. You can buy a SUV today that will do much better than 20 MPG nominally for daily driving, but it will not get 20 MPG towing the lightest, smallest RV that includes toilet and shower (and might cost about $12,000 to $16,000).
Owning a RV for ten years that averaged 8.3 MPG over 30,000 miles, fuel costs ($9800) have been trivial compared to ownership costs ($50-60,000 including depreciation, insurance, storage, maintenance and loss of income on the money spent for the thing). If you are worried about costs, don't think so much about fuel costs, think about cost of ownership instead. That means the least expensive RV you can live with, not matter how much fuel it takes to move it around.
well put.
