I would do 2 things.
1. Contact the dealer and ask him to mail you the spacer kit that came with the hitch. They were all there at one time.
2. If you need to use the hitch until the dealer supplied ones show up, get some grade 8 flat washers and use them.
The fine thread bolt on the bottom is to take up the play of the assembly when you have the hitch head loosened up from the shank. The bolt puts some preload on the upper pin with the spacers until you torque up the 3/4" bolts on the shank. This way the pin is dead tight agasint the shank before you tighten the 3/4" bolts.
