I had the sheath come lose on mine, right at the handle. I pulled a small amount of the underbelly down and could see the whole cable "bow" when the handle was pulled or pushed. I got a short piece of metal (actually one of those saw toothed picture holders) and put small hose clamps on it to secure it to the handle and to the sheath. It's been working fine since last April.
Maybe yours is the same failure as mine. If so, this fix seems to work.
