Assuming we are talking about a limited access freeway ramp and there wasn't some special condition you didn't include in your description...what's up with that is an incompetent Traffic Engineer (fyi: I am a Traffic Engineer).
Was this in a construction zone with traffic basically at a standstill? Was there a little traffic signal on the side of the road? I really hope there is more to the story as that's just an accident waiting to happen.
