There are no safety chains on 5th wheels. Just the emergency brakes to stop the 5th wheel during separation from tow vehicle.

My bad, I forgot the 5er hitch does not have them. My brain went the the goose-neck hitch as my truck bed has the B&W turn over ball hitch in it for gooseneck trailers which does use the chains so that came into my mind the 5er had them too. Not so.
Hopefully we can learn what actually did let go.
