I've never disassembled my B&W hitch, so I cannot categorically state that the jaws cannot open when closed and pinned. The only people who can state that are the ones who have actually taken a hitch apart and examined all components. Pivot pins can wear and break, latches can fail, but there should be evidence of what happened. All we can do is hope the OP comes back with an explanation.
