The trailer coming unhitched, mysteriously, is not related to installation of the hitch, since the hitch is one gigantic assembly that drops down in to the truck bed.
Is still makes no sense how the trailer could come out of the hitch, yet the hitch jaws are secured and the latching bar is still engaged properly.
Photos should be sent to B&W so they are aware of this scenario. If it were me in your shoes, I'd want someone from B&W to fly out to your installing dealer and look over the hitch, then I'd want a new hitch, because there's no way I'd trust that one again.
