I will have the hitch and the installation reviewed for faulty parts or improper installation, before I use it again so I can be reassured that this will not happen again. Although the investigating patrolman indicated the installation appeared to be correct and was extremely solid and secure. As an engineer, I know that nothing is fool-proof. New parts sometimes fail and human operators occasionally make mistakes.
Just curious but who is doing the "reviews" and hands on decision making if the hitch is in proper working order.
Has B&W requested the hitch be sent to them or are they going to let the B&W dealer in your area handle their end ??
