I've been using a B&W Patriot for 4 seasons. Never one issue. Maybe it's just me but unless you do something WRONG I see no way it would ever unhitch. If you back in and have the pin low enough that you push the truck down a bit when hitching, then there's no way you're not hitched.
The handle is in the open position when hitching, as you back in the handle automatically slide to the closed position. Stick the safety pin in and you're done. After 4 seasons of doing the above, it's always worked. When I'm done pinning the handle I walk around to the back of the truck, look at the jaws and then plug my trailer plug in. Never do a pull test. I did when I 1st got it, but I saw no point after awhile.
The OP must not of pinned the handle right or something physically broke inside the 5th hitch.
The system is foolproof "IF" you do as you said. I have used a Companion since 2005 and did the pull test as I faithfully did with my RBW but like you I changed to a "VISUAL" test. 11 years and many thousands of miles towing HEAVY not an issue.
It really amazes me with all the "what if's" in the world. Really kinda scared to tow now! ;)
