I think we all are on the same page about one thing: we aren't a jury deciding a legal outcome, we all just want to know what happened so we can make sure it doesn't happen to us or anyone else again.
Redundant latches are a good thing. On bumper-pull type trailers there is often discussion about using weight distributing hitches and few ever mention what I consider one of the best reasons to use one: since their design forces the coupler down tight against the hitch ball, they make it virtually impossible for the trailer to come off the hitch even if you forget to latch the coupler. I use mine even when I don't need it for weight transfer.
I've always been surprised that 5th wheels don't have safety chains.
It seems to me that having lock jaw geometry that forces the jaws closed the harder the pin pulls on them is good engineering for the real world of humans forgetting things.
