My thoughts are that it was either
1 high hitched or
2 the latch handle wasnt fully closed when the operator installed the lock pin so that the lock pin wasnt through both holes or
3 the latch handle wasnt fully closed & locking pin was installed in the hitch part but not the latch handle or
4 the king pin was in front of the locked and pinned hitch jaws
I dont see how its possible for the rv to separate , then the hitch jaws close back up & re-lock and the lock pin re-install itself. We are not getting the full picture here.
