I would not try to fix that.
That is a structural problem with the slide.
I would want Keystone to take that cracked board out and replace it with a good solid one.
Doubtful the top bolt "fell out". The bottom one looks like a lock nut. They don't just come undone. There was never one put there to begin with.
OP see if there are two bolts on the side.
