99% of the jack knife occurrences can be attributed to one of three things, poor driver control, overloaded/incorrect loading of the trailer, or the load was too much for the TV.

Over loading will always get you in trouble, in one way or another.
And the last of the bunch is not enough TV for the job. specially when the driver compensates with equipment like a load equalizing hitch. In reality equipment malfunctions leave you in a position for an accident.
I look back on when I was a kid towing a 2 ton hay wagon with a military surplus jeep, and wonder why I didn't get killed
