I'm not sure where you got the idea that you don't need a medical for an Ontario Class D license, but it's wrong. A medical IS required.
Medical required for Ontario Class A and D LicensesI suspect that this is another case, like Florida telling Canadians they were going to have to get an International Drivers License, where the nice folks setting the 'new' policy forgot that there an 'OLD' agreement between Canada and the USA that says otherwise.
It's also covered by NAFTA which was ratified by both governments at the Federal level and became law in both countries.
