Having spent about 45 years in quality control and inspection of mechanical products, I can assure you that Murphy is alive and well. Anything that CAN go wrong, WILL. Sooner or later. No matter how much time and money you spend on any type of quality control/assurance program, you can only reduce the error rate. You cannot make it zero. The problem the OP shows is amusing, unusual, and makes the product unusable. Luckily, no one will die because of it, and it is easily remedied by repair or replacement.
