Sounds like someone latched the control lever down after the pump was turned off. The operator selected the grade and the pump started dispensing fuel before the man had the nozzle inserted into the filler neck. The man could have still been turning toward the RV spraying fuel as he turned.
Some thing to check just like making sure no one has messed with your hitch while you were away.
All nozzles I've used won't do what was reported in the story. If the nozzle latch is set on full pour, as soon as the pressure is removed from the previous pour, the nozzle returns to the closed position, i.e. no gas can pour if the filler hose is pressurized again,the user will have to unlatch the device by squeezing on the handle.
