Sounds like someone latched the control lever down after the pump was turned off. The operator selected the grade and the pump started dispensing fuel before the man had the nozzle inserted into the filler neck. The man could have still been turning toward the RV spraying fuel as he turned.
Some thing to check just like making sure no one has messed with your hitch while you were away.
